State Championships - Player Exemption Policy & Form
The purpose of this policy is to provide guidelines and an official form for the request for an exemption to attend trials for Regional Team selection or make selectors aware that they are unable to attend the State Championships.
(NB: Players will also need to complete the State Championships Nomination Form)
Policy:
It is acknowledged that players who are unable to attend trials for whatever reason may still be eligible to be selected for Regional and State Teams.
Approval from Hockey Tasmania regarding requests for exemption to trial is provided to ensure that there is transparency in the process of selection.
Procedures:
- This form is to be completed for the purpose of requesting an exemption to attend selection trials.
- Requests must be lodged with Hockey Tasmania as soon as possible of being notified of the trial dates and times.
- All exemptions are required to be submitted prior to date of trial.
- Where an exemption is requested for health reasons, ensure this form is lodged prior to the trials. A medical certificate should be provided to the State Teams Coordinator
- Hockey Tasmania will provide your Exemption details to the relevant coach/selector.
